Short Term Rehab/Subacute Care

Our dedicated unit for Short Term Rehab and Subacute Care offers high staffing ratios, RN case managers and attending physician services, supplemented by a physiatrist and physician assistants. Our highly qualified rehabilitation therapists are assisted by an orthopedist providing inservice training and quality assurance review. Comprehensive short term rehabilitation, including physical, occupational and speech therapies, is available 7 days-a-week at Garden Care Center. With a team approach, a treatment plan is customized for each patient in order to maximize their potential for recovery and independence and a swift return home.

Our interdisciplinary team, comprised of doctors, nurses, case managers and dynamic caring rehab staff, develops each resident’s plan of care with the goal of attaining quality outcomes. Residents and family are key members of the team. In addition to medical diagnosis and history, prior level of functioning needed to attain independence or other personal goals are considered.

The spacious rehab area has a full working kitchen, modified bedroom and training bathroom to prepare residents to return home. Our rehab equipment includes a treadmill, electric parallel bars, arm bike for upper extremity exercises, stationary and recumbent bike and a training staircase. Numerous modalities are available to treat pain, inflammation and improve blood flow and flexibility. We are especially pleased to offer a Cybex, full circuit weight system and a Neurocom balance master which is a fully computerized balance system to aide in diagnosing and treating balance and vestibular disorders.

In Physical Therapy, walking, balance and muscle strengthening are emphasized with the goal of returning home and reducing falls. Our orthopedic rehab program follows the recommendations of your orthopedist or in the case of joint replacement, your surgeon. Our cardiac rehab program follows established protocols in conjunction with the facility’s cardiologist. Amputee and prosthetic management training is done by skilled, caring professionals, highly qualified to deal with these challenges.

Occupational Therapy improves the resident’s independence through working on their activities of daily living such as eating, dressing, toileting, personal hygiene and ability to position oneself in bed. Range of motion for upper extremities, muscle strength and tone and when needed, the use of adaptive equipment are all emphasized. Residents in wheelchairs are taught, independence in mobility and safety awareness.

Speech and Language Therapy is available to facilitate communication, which is so important in maintaining quality of life. A complete speech, language and dysphagia evaluation is administered, if needed, and a treatment plan is developed with the goal to restore each resident to their maximum potential.
