Outpatient Services
Our facility takes great pride in the rehabilitation services we provide to the residents living within our facility. We believe rehabilitation is a dynamic process of planned adaptive change in life-style responding to unplanned changes imposed on the individual by disease or traumatic incident. The focus is not on the cure, but on living with as much freedom and autonomy as possible at every stage and in whatever direction the disability progresses.
In an effort to extend these same services to persons with disabilities residing within the community, we have developed an outpatient rehabilitation program. This program is designed to meet the rehabilitative needs of community persons who have been identified as having potential to improve their current level of functioning and their attending physician has recommended either Physical, Occupational, or Speech Therapy Services.
Patients living within the community, upon their physician’s order, will be evaluated by licensed rehabilitation staff, and a treatment plan including goals, interventions and time frames is developed. This evaluation, as well as every aspect of the treatment plan, is completed within our facility’s rehabilitation department by our rehabilitation staff.
Our philosophy of care is to assist all patients in attaining or maintaining their highest practicable level of functioning. We believe individuals can achieve optimum independence within the limits of impairment, provided they are included in decisions about the rehabilitation plan. We promote equal status for each patient as a participating member of the rehabilitation team.
We deliver the highest quality person-centered rehabilitative outpatient care services and facilitate an individual’s movement toward improved health. Our approach to rehabilitation is holistic, creative, caring and optimistic.
